Case Study:Amblyopia1)
Ms Li,female,7 years old
Main complaint:Ms Li,a recently adopted 7-year-old female,was seeing a pediatrician2),Dr McLaren,for the first time.Her new family was concerned that she might have visual problems resulting in self image and schoolwork issues as one of her eyes appeared to deviate inward.Her physical examination was unremarkable except for the eye examination.Dr McLaren explained to the parents that Ms Li had a condition known as strabismic amblyopia3),or a “lazy eye,” and made a referral to an ophthalmologist4).
Examination:Upon examining the patient,the ophthalmologist noted that the left eye deviated toward the medical canthus(angle).A complete visual examination was conducted,and the diagnosis was confirmed.Ms Li did have amblyopia,in which one eye has lower visual acuity5) and is used less than the other eye.She aslo had slight hyperopia6),commonly known as farsightedness.A treatment plan was devised and directed toward the development of normal visual acuity.It was discussed with the parents,who decided to move forward with the therapy.
Clinical Course:The ophthalmologist explained to the patient that they wanted to make her weak eye stronger so she would see much better.This would be accomplished by putting a patch7) over the strong eye,which should correct the deviation.She would need to wear the patch for a prescribed number of hours each day,and she would also need to wear glasses.She would need to return to see the ophthalmologist so progress could be measured.While Ms Li was not sure of the patch,she was excited about wearing glasses since her new mom and sister also wore glasses.She was fitted for glasses and provided with the “band aid” type of patch8) to apply over her right eye.
Follow-up
Ms Li started wearing the patch on her right eye during waking hours.She progressed to wearing it four to five hours a day as ordered by ophthalmologist.The glasses she obtained from the optician were helping her to focus,and she was able to read her schoolwork.She had adjusted well to the treatment plan and showed improved vision.The family was satisfied with results from the therapeutic plan.
