50年前的中国处在一个众所周知的特殊历史时期,政治、经济万马齐喑,在医学、科研方面唯一的一线光明是周恩来总理发出“一定要攻克肿瘤”的号召。于是医院抽调了几位各科的医生,以汤钊猷医师为首组织了“肿瘤(研究)小组”,我亦忝列其中,未几改称“肝癌(研究)小组”,明确以肝癌为研究对象,并将其时一个内外科合作管理的“工农兵病房”作为临床研究基地,专业收治肝癌病人,于是有了余业勤、林芷英、陆继珍、周信达、唐辰龙诸医师的加入,形成了肝癌研究所最初的雏形。
其时中山医院决定以肝癌为肿瘤研究的重点,也是因为从20世纪50年代开始,中山医院即在林兆耆院长主持下开始有了初步的肝癌临床研究工作。
肝癌(研究)小组成立后,限于当时的水平和条件,事实上也只是做些临床医疗、做点泛泛的总结罢了。由于其时肝癌的病因不明,无法预防,也无法早期诊断,收治的病人几乎全部都是晚期病例,能手术切除的极少,使用一种名为AT581或AT1258的药物研究所试产的氮芥类抗癌药注射也基本无效,肝癌确属“不治之症”。大家能做的只是对病人洒一腔同情之泪。
20世纪的70年代初,甲胎蛋白开始应用于肝癌的临床诊断,在汤钊猷医师的领导下,一系列的临床研究拉开了序幕。很快我们便将甲胎蛋白用于一般工农群众的筛查,开了肝癌早期发现的先河。一批早期发现的无症状病例,获得了手术切除,开创了“亚临床肝癌”的研究。为了探索肝癌的定位诊断,扩张了脐静脉做造影检查;为了研究不能切除肝癌的局部治疗,探索了冷冻治疗、激光治疗;开展的肝动脉结扎、插管化疗事实上是如今普遍使用的介入治疗的前奏;为了探索免疫治疗,使用了卡介苗,等等。在一系列的综合治疗下,肿瘤缩小了,于是又有了“二步切除”的研究……
早期的临床研究在今天看来可能粗糙了些,但这些临床型的研究却多属原创性的研究工作,因为当时即使在发达国家亦无此类系统研究。原创型的研究需要主事者对医学科研规律的洞悉和掌握,临床型的研究需要主持者对研究实施的把握和担待。在这一点上,肝癌研究组乃至以后的肝癌研究室、肝癌研究所的汤钊猷医师(教授、院士)恰是不可多得的领导。汤院士擅于发现苗头、擅于提出新的目标、擅于组织研究力量、擅于总结提高、擅于解释取得的成果。复旦大学肝癌研究所能发展到今日之规模,创造出今日之成就,汤院士功不可没。
肝癌研究所半个世纪的发展,得“天时、地利、人和”之机缘,天时指新中国成立,指改革开放40年的大好时机;地利指上海本是肝癌高发地区,指原上海医科大学今复旦大学,更是指中山医院的各级领导、各有关部门与科室给予的支持和配合;人和指肝癌研究所内科与外科、医生与护士、临床与实验室、老师与同学、老中青三代人的团结协作。肝癌研究所的成就得之于天时、地利、人和,但从具体的工作看,“人的因素是最重要的”,汤院士擅于团结大家共事,加以肝癌研究所全体工作人员齐心协力,在医疗、教学、科研各方面努力奋斗是事业取得成就的根本。
肝癌研究所所取得的成就是中国特色医学科研的缩影,肝癌研究所所取得的成就也是大学附属医院临床与科研工作结合的典范。我以我亦是肝癌研究所的一员而自豪。我祝愿肝癌研究所在樊嘉院士的领导下取得更加辉煌的成就,也祝愿汤钊猷院士健康长寿。

严谨创新、砥砺前行——贺复旦大学肝癌研究所成立50周年暨汤钊猷院士从医执教65周年
时光荏苒,复旦大学肝癌研究所成立50年了。我衷心祝贺研究所50华诞!
肝癌研究所是顽强的,她成立于科学研究工作陷入停顿、医疗秩序陷入混乱的时期。
肝癌研究所是幸运的,她50年的历练中有40年处于改革开放的新时代。
肝癌研究所是中山人的骄傲。她是中山医院机体的重要组成部分,和中山医院同甘苦、共命运,为医院的建设和发展作出了突出的贡献。她和心血管病研究所被戏称为“心肝宝贝”。
复旦大学肝癌研究所的成就,学术界人人称道,社会上家喻户晓。我本人也深受汤校长和杨院长教诲,领略过余业勤等老一辈专家们的风采,永不能忘。
现代医学的医疗、教学和研究工作是一项庞大的系统工程,既有有形机构,更是无形的系统。建设这种系统工程的难度毫不亚于在崇山峻岭架起一座飞越天堑的大桥,也不亚于创立一家誉满天下的大型高科技企业。这项工程是由各方面人才组成,是一个充满集体智慧和劳动的系统。她对医学和社会的贡献是无法替代的。
一个经历了50年风雨形成的集医疗、教学和科研的超群集体,一定有既高瞻远瞩又脚踏实地的带头人。研究所几代人成就了他们的杰出代表,先后产生了汤钊猷和樊嘉两位院士和大批专家,为各地肿瘤学领域培养了数以千计的专门人才,不愧为优秀的研究机构和上海市临床医学中心,称得上中山医院学术上的“半壁江山”。
肝癌研究所和中山医院的老师们比我更加了解研究所的历史和医、教、研成果。因此,我不打算全面回顾和罗列研究所的历程和取得的具体成绩,也不试图全面总结提炼研究所的经验。
在20世纪七八十年代十分艰苦的条件下,先驱们到肝癌高发区做流行病学调查,开创了研究工作。70年代初期在医院层面成立了肝肿瘤内科和肝肿瘤外科,这给肝肿瘤的综合临床研究提供了新的空间。2002年成立的上海市肝癌医学中心,把肝癌的医疗、教学和研究工作提高到一个新的层面。研究所艰苦的工作条件一直延续到2015年东院区投入使用才得以根本改变。
40年前我刚到中山医院的时候听说有个肝癌研究所,心中不由得冒出一个疑惑,为什么上海第一医学院在有肿瘤医院的情况下,中山医院还要成立一个独特的肝癌研究所。我请教过不少同事,却没有得到满意的答案。经过我40年的观察和有幸参与,研究所的实践和成就不但逐步解开了我的疑惑,而且让我体会到前辈们50年前的决策是多么有远见。综合性教学医院各学科同事的有机合作、互相激励是全面建设领先专科的最佳生态。
血管外科研究室于1978年成立之初,因为条件有限,一时没有办公场所。汤钊猷所长慷慨地挑选研究所3楼位置最好的一间实验室给冯友贤主任使用。我们自称肝癌研究所的“特区”,成了一段中山医院研究工作历史上的佳话。
50年来研究所始终瞄准肝癌研究的目标和中心任务。当然,肝癌单病种必然涉及肿瘤学的全部领域,可以说肝肿瘤的研究实际上是肿瘤学的研究。汤钊猷教授曾经概括说做了两件半事:第一件是早期发现肝癌;第二件是把大肿瘤变小,然后切除;第三件是防止肿瘤复发,因为还没有完全解决,所以只算半件事。几代人共同努力,达到肝癌研究工作今天的高度、深度和广度。
经过全国同行几十年的共同努力,乙型肝炎和原发性肝癌的发病已经在下降,但是远远没有消灭。肝癌仍然严重困扰着相当大的人群。同时,转移性肝癌变得相当常见。在肿瘤学研究的道路上仍然任重道远,有无数的未知领域有待认识,有无数的难题有待攻克。我作为在中山医院工作40年的老职工和曾经的管理者,期待着复旦大学肝癌研究所的同仁们继续发扬“严谨、求实、团结、奉献”的精神,继承发挥肝癌和肿瘤研究的先锋作用,不断攀登,为更多的病人解除痛苦,为人民医学卫生事业做出新的贡献!

Dear Professors Zhao-You Tang,Jia Fan,Jian Zhou and Hui-Chuan Sun,
It is an enormous privilege to have the opportunity to send a message about the 17th National Liver Cancer Conference and the 1st Shanghai International Liver Cancer Forum.This meeting has come to fruition due to the evolution of the Liver Cancer Institute at Zhongshan Hospital,Fudan University which is universally accepted as one of the best in the world.This evolution has been driven by a uniquely talented and hard working group of Professors who have achieved a prodigious amount becoming world leaders in their fields including basic and clinical research,surgical technique,surgical results pushing forward the boundaries of what is possible and prolonging the lives of their patients and finally the training of a huge number of junior surgeons who themselves have gone on to become leaders who are internationally recognized.
This year's event is unique in representing not only fifty years of development of the Liver Cancer Institute under the outstanding tutelage of Professor Zhao-You Tang and Professor Jia Fan but also allows many,many surgeons to celebrate the contribution of Professor Zhao-You Tang who for an unbelievable 65 years has taught and guided several generations of outstanding surgeons and leaders.Professor Zhao-You Tang's longevity and the sheer volume of research and clinical work have ensured that he is one of the world's leading surgeons and someone who is known and revered in all HPB units and by all HPB surgeons from every corner of the globe.It is almost impossible to imagine how someone can remain at the forefront of surgical science,research and training for that length of time and if it were not for decades of accolades from his pupils and his international presence many people would beyond many peoples understanding.(https://www.daowen.com)
Professor Zhao-You Tang's influence and guidance is such that he has inspired further great surgeons and in Professors Jia Fan,Jian Zhou and Hui-Chuan Sun the direction and further progress of the Liver Cancer Institute at Zhongshan Hospital is assured and he is able to hand the baton to a new generation of leaders.
Under the guidance of Professors Jia Fan,Jian Zhou and Hui-Chuan Sun the Liver Cancer Institute at Zhongshan Hospital has flourished and developed at an unbelievable rate and the volume of clinical activity they have achieved would seem impossible to many ordinary HPB surgeons.This volume of work continues to be allied with cutting edge basic science and the development of new techniques in surgery,oncology and radiology and a tissue bank that is the envy of every surgeon in the world.The research output and the presence of that research in the highest impact factor journals attests to the quality and the results and conclusions that can only be made possible by the outstanding patient outcomes and the numbers of patients available for study.
I consider myself extremely fortunate to have been able to visit Zhongshan Hospital,to see first-hand what is possible when such talented teachers are involved,demonstrating practice changing surgical techniques,challenging accepted dogma and explaining how it is possible to combine this with a world leading research unit.It is clear however that the present position and the international reputation could only have been achieved through extraordinary vision and an unrivalled capacity for hard work and in the future can only result in even greater prominence for this world class unit.
Everything I have seen when I have visited the Liver Cancer Institute at Zhongshan Hospital has humbled and inspired me and particularly the professionalism and the ability of leaders to build such a close knit team despite the clinical pressures.I consider myself privileged to be able to include these individuals in the group of my most valued friends.

I always remember the great moment of my first meeting with Pr Zhao-You Tang.It was in the early seventies.Shanghai was a sad city without cars and hotels,(I was hosted in the hospital).But so rich was the exchange that I had during 2 days with Pr Zhao-You Tang on his extraordinary experience in the field of liver surgery in liver tumors.
Since that time I came several times at the Liver Conference organised by Pr Zhao-You Tang.I remember during one conference that ceremony when a choral of dozens of patients living more than 10 years after surgery for their liver cancers sang in homage to their surgeon.
Pr Zhao-You Tang is a great surgeon,researcher and teacher and his contribution to the knowledge and the treatement of liver tumors especially hepatocellular carcinomas is recognized all over the world.

Professor Zhao-You Tang—A Pioneer In The Pathogenesis,Diagnosis,and Early Treatment Of Hepatocellular Carcinoma
The introduction of the concepts of the need for early HCC diagnosis,screening and treatment have been profound and established the Zhong Shan Hospital and the Shanghai First Medical College as an international leader,under the guidance of Professor Zhao-You Tang.Many of the concepts,including the stepwise progression of tumor invasiveness,the role of liver inflammation and tumor biology,and the feasibility and techniques of liver resection were introduced at a period when little was known about HCC and hepatitis B.The principles outlined in the early writings by Prof.Tang,are as valid today as they were,when first introduced in 1979.In fact,even without modern day tools,such as molecular biology,tumor immunobiology,antiviral therapy,conformational radiotherapy and liver transplantation,Prof.Tang demonstrated that limited resection is feasible in patients with chronic liver disease and cirrhosis,that screening for HCC is possible,and importantly that inflammation is a fertile ground for tumor development and growth.The advent of modern technology has only validated the approaches Prof.Tang outlined more than 40 years ago.
Prof.Tang is an internationally recognized surgeon and scientist.He has trained several generations of surgeons,physicians and scientists from China and around the world.His contributions,including over 500 publications in internationally acclaimed peer-reviewed journals attests to a lifetime commitment to the understanding and treatment of this leading cause of death in Asia.
On a personal note,I am honored to be able to be joining this notable gathering in celebration of the life and achievements of this outstanding surgeon,scientist and friend.He received me as a visitor to his unit in 1986,at that time accompanied by my parents on my first trip to visit China.Prof.Tang graciously gave me a copy of his book“Subclinical Hepatocellular Carcinoma”,which I keep on my desk.

Over the past five decades,the Liver Cancer Institute of Fudan University,headed by Professor Tang,has played a vital role in nurturing professionals,imparting knowledge,and providing specialized care and treatment to liver cancer patients.For over 30 years,Professor Tang has devoted his research on hepatocellular carcinoma which has resulted in great improvement in prognosis of this disease.His life-long contributions to teaching,research and medical services in the field of liver surgery have gained him national and international recognition and fame.

Congratulations 50th anniversary of Liver Cancer Institute, Zhongshan Hospital,Fudan Universit, and Congratulations Professor Zhao-You Tang's 65th anniversary of practicing and teaching medicine!
At the end of 1970's, I was visited Shanghai because Professor Tang published many papers on hepatocellular carcinoma and also a book entitled“Small Hepatocellular Carcinoma". I was really impressed by his book, and respect him there after.
Thanks for Professor Tang's consecrations on the treatments of liver cancer. I hope that this is just the beginning of many new achievements you will acquire throughout the future.
