7.1 TYPESOF FORMS
Application Form.An application form is a type of widely used form that requires certain facts to be stated briefly and exactly.
An application form usually includes contents as follows：
●Personal data such as name，sex，native place，marital status，health，contact information；
●Specific requirements on the applied items（a school，a course，a post...）；
●Declaration；
●One's signature，usually hand-written.
Registration Form.A registration form is used widely for various purposes，such as arrival/departure，enrollment，employment，temporary residence.
A registration form usually includes contents as follows：
●Personal data of the one who is to fill out the form，including name，date of birth，occupation，native place，ID number or passport number；
●Cause of registration（e.g.on business trip，for study abroad）；
●Contact information；
●Declaration；
●Signature.
Different forms are different in requirements.While designing a form，you should decide on the following items：
