Annex 2：自我维持装备月报表

Instructions for filling in the above form:

1.Fill in the green shaded areas in the above report.

2.The above report reflects the status of the unit’s Self-sustainment Equipment assessment as at the date of the last formal COE unit inspection.

3.If the Self-Sustainment status of the unit has not changed，the CO should sign the form and pass it to SHQ for staffing with the corresponding Major Equipment Monthly Report.

4.If the CO feels that the status of any category has changed，the unit should immediately contact the COE unit who will arrange a reassessment and submit the necessary reports.

5.After completion of the form，indicate the Rank and Name of the CO who must sign the document before submission to the relevant SHQ.
