Introduction to Henan cancer registration system
Cancer registration report is a statistical system for constant collection, storage, sorting, statistical analysis to evaluate cancer incidence, mortality and survival data in the target population according to the unified technology and method.Cancer registration is generally acknowledged as a standardized method for collecting information on cancer incidence all around the world.It aims to know cancer incidence,mortality and survival in the population, and to provide scientific basis for cancer etiology study,health policy development on cancer prevention and control, and for the relevant practice effect evaluation and assessment on cancer prevention and control.
In the late 1950s, The Henan research cooperative group were mainly from Henan Medical College,Henan College of Traditional Chinese Medicine,and Henan Provincial People's Hospital.They penetrated into Lin County（now Linzhou City）mountainous areas and carried out esophageal cancer epidemiological investigation to explore cancer prevention and control strategies.From then on esophageal cancer incidence and mortality registration was initiated in Lin County.During 1959 to 1962, the information was mainly collected depending on retrospective survey.During 1963 to 1966, esophageal/Cardiac cancer registration and reporting system had basically spread in the three administrative levels of county, township and village.During 1967 to 1969, cancer registration system was severely affected by the“Cultural Revolution”and many cases had been reported under surveillance.Anyhow, the missed cases had been supplemented later through retrospective investigation.In 1977,Henan Provincial Health Commission organized a retrospective survey on all causes of death from 1974 to 1976 with a coverage of more than 60 million population according to the national deployment.
Whereafter, in order to know all the deaths caused by cancers and other causes, Henan Cancer Hospital / Henan Cancer Institute established cancer-centering vital statistics in 1983.It covered one tenth of Henan provincial population, comprised 15 selected counties / cities, Lin County, Jiyuan City,Yuzhou City, Luoyang City, Yanshi City, Luoshan County, Lushan County and Neixiang County.
1988 年，林县在全死因登记系统的基础上，参照国际癌症中心（IARC）/国际癌症登记协会（IACR）推荐的肿瘤随访登记方法与原则，建立起了以人群为基础的肿瘤发病登记报告系统，对恶性肿瘤及中枢神经系统良性肿瘤进行登记报告和编码。2004 年4 月1 日，河南省癌症及生命统计中心成立，并在15 个市（县）建立了癌症及生命统计登记处。2004 年10 月7 日，河南省卫生厅印发《河南省癌症及生命统计登记工作规范》的通知，下发到市、县卫生局及各登记处并落实执行。
2008 年，河南省肿瘤医院/研究所在偃师、禹州、内乡、开封、鲁山开始实施人群肿瘤登记报告工作。2009 年，国家卫生部在全国范围内启动了中央财政转移支付地方的肿瘤随访登记项目工作，中央财政对登记市县给予经费支持。河南省的林州、洛阳、偃师、禹州、西平和沈丘纳入国家肿瘤随访登记项目市县。在2009 年的基础上，2010 年将济源市、鲁山县、内乡县、郸城县、罗山县、漯河市、三门峡市、虞城县列入河南省肿瘤登记系统。2012~2013 年又将开封市、睢县、鹤壁市及郑州市纳入全省肿瘤登记系统。
为进一步加强全国肿瘤登记工作规范化管理，获得及时、统一、准确的肿瘤发病、死亡和生存信息，2015 年2 月5 日国家卫生健康委员会和国家中医药管理局联合印发了《肿瘤登记管理办法》，河南省卫生健康委员会和河南省中医管理局结合我省实际情况，组织制定了《河南省肿瘤登记实施方案》（豫卫疾控【2015】10 号）。河南省肿瘤防治研究办公室按照《河南省肿瘤登记实施方案》的要求对全省各级医疗机构相关人员进行了业务培训，要求各地市和省直管县成立肿瘤登记处，开展肿瘤随访登记工作，并通过河南省网络直报系统上报当地的肿瘤发病、死亡和生存随访数据。
各级肿瘤登记处履行肿瘤登记报告职责，建立健全肿瘤登记报告工作管理制度，明确肿瘤登记报告管理科室及管理人，明确诊疗机构科室责任。
In 1988, the first population-based cancer registration system was established based on the vital statistics system in Lin County.Under this system,all cancer registration and follow-up principles were refered to International Agency for Research on Cancer (IARC) / International Association of Cancer Registries (IACR), and all cancers (including central nervous system benign cancers) were classified and reported by the codes in accordance with the International Classification of Disease.On April 1,2004, Henan Provincial Cancer and Vital Statistics Center was set up.Meanwhile, 15 city / county Cancer and Vital Statistics Registries were opened.On October 7, 2004, Henan Provincial Health Bureau issued the“Methods for Cancer and Vital Statistics Registration, Henan”, which was sent out to each registry as work specification.(https://www.daowen.com)
In 2008, Henan Cancer Hospital / Institute built the population-based cancer registration and reporting system in Yanshi City, Yuzhou City,Neixiang County, Kaifeng City and Lushan County.In 2009, the registries could be supported by the central government finance, as Ministry of Health launched the transfer payments on cancer follow-up registration program to local governments.In Henan province, the funded registries located in Linzhou City, Luoyang City, Yanshi City, Yuzhou City, Xiping County and Shenqiu County.The funded registries were added up gradually.In 2010, Jiyuan City, Lushan County,Neixiang County, Dancheng County, Luoshan County,Luohe City, Sanmenxia City and Yucheng County were added, and Kaifeng City, Sui County Hebi City and Zhengzhou City were contained during 2012 to 2013.
In order to strengthen cancer registration specification to obtain accurate, prompt and unified cancer incidences, mortalities and survival data,the National Health Commission and National Administration of Traditional Chinese Medicine issued “Cancer Registration Management and Methods” on February 5, 2015.Henan Province Health Commission and Henan Province Administrative Departments for Traditional Chinese Medicine schemed out “Cancer Registration Management and Specifications” applicable to Henan Province.Correspondingly, Office for Cancer Control and Research, Henan trained all the staffs who worked in cancer Registries or the relevant medical institutions regarding to the cancer follow-up and reporting management specifications and Henan Cancer Reporting System, which was the dedicated network for the registries to report all the cancer incidences, deaths and survival.
从早期林县（现林州市）食管癌/贲门癌的单癌种登记系统，发展到目前的全部位的肿瘤随访登记系统，是为了满足肿瘤预防控制工作的实际需求。肿瘤随访登记工作以政府为主导，河南省卫生健康委员会作为项目主管单位全面负责肿瘤登记报告工作，制订相关政策和规划，组织开展考核和评估。
河南省肿瘤医院、河南省肿瘤防治研究办公室作为技术指导单位，负责全省肿瘤登记系统的建立和工作的实施；制定及修订肿瘤登记报告实施方案和年度工作计划；负责开展全省肿瘤登记工作业务培训、技术指导等工作。
河南省各市、县（区）卫生行政部门负责辖区内肿瘤登记报告系统的建立及项目工作的管理和实施；对肿瘤登记报告工作进行督导检查和考核评估；协调相关机构和部门，为肿瘤登记报告工作的正常运行提供保障。
市、县（区）肿瘤登记处负责制定本辖区肿瘤登记报告工作计划和实施方案；指导辖区内各级医疗机构开展肿瘤登记报告工作；确定辖区内肿瘤登记报告单位、报告人、联系方式、并登记建档；负责肿瘤登记的业务管理和技术指导等工作。
各级医疗机构为登记病例的上报单位，要求各级医疗机构在当地肿瘤登记处的指导下，利用河南省肿瘤随访登记网络直报系统开展肿瘤登记报告工作，按照项目内容要求，提供及时、准确、完整的肿瘤登记信息。建立、健全机构内的登记报告管理组织制度和技术规范，将资料登记及死亡报告工作纳入本单位内部考核管理内容，并制定相关的管理措施。
