3.2 Invasive Ductal Carcinoma
The term:Invasive ductal carcinoma,non-special type, tumor cells break through the basement membrane;lack of myoepithelial cells
Clinical manifestation:
●Breast mass,with or without pain
●Abnormal nipple(effusion,shrink,eczema)
●Regional lymphadenopathy
Population distribution characteristics:
●The peak incidence is after 40 years old
●The disease is the most common type of invasive cancer and accounts for 40%-75%of invasive breast cancer
Mammography:
●Irregular/high-density lumps of marginal burrs, infiltrates or small lobes:31-43%with calcification
●Diffuse or localized trabecular thickening of the breast(edema),with or without skin thickening
●Focal asymmetry,Structural distortion
MR:
●T1W1:Multi-present signal,T2W1 fat suppression phase:Signal level is different, mostly equal or slightly lower signal(https://www.daowen.com)
●DWI signal increase,ADCreduce
●Hemodynamics:Early rapid and obvious reinforcement, latency is gradually or quickly cleared
Image inspection recommendation:
●Best image inspection method:
☆Screening:Mammography±DBT(Mammary digital tomosynthesis imaging)
☆High-risk groups need to be combined with breast MR screening
☆Initially examined age<30 years old, pregnant or symptomatic lactating patients should choose ultrasound
