7.2.3 Sorafenib(Sorafenib,BAY43-9006)
Sorafenib is a multi-targeted serine/threonine kinase inhibitor with targets including c-raf and b-raf as well as RET,Flt-3 and c-kit.Sorafenib also has strong activity in tyrosine kinases, including the VEGFR family(VEGFR1~3)and platelet-derived growth factor receptor(PDGFR),which are important receptors in tumor angiogenesis. Therefore, the anti-tumor activity of sorafenib in the body is directly inhibited by the Raf/MEK/ERK pathway on tumor growth;the second is through the anti-angiogenic effect of the drug. For the treatment of metastatic renal clear cell carcinoma, advanced liver cancer and thyroid cancer.(https://www.daowen.com)
The single-agent sorafenib did not show strong activity in metastatic breast cancer. The solitif-0701 phase Ⅱ clinical trial evaluated the efficacy of sorafenib in combination with capecitabine in the progression or metastatic triple-negative breast cancer.229 patients were enrolled. The results showed that although there was no difference in OS,the median PFS(6.4 months vs 4.1 months,p=0.001)and ORR(38%vs 31%,p=0.25)increased and good tolerance,Adverse reactions such as rash,diarrhea,mucositis,neutropenia,hand-foot syndrome, etc.Resilifence phase Ⅲ clinical studies have shown that sorafenib can not prolong the PFS and OS of HER-2 negative breast cancer patients.
