十三、预后

GD总体预后良好。ATD治疗的缓解率差异很大,为20%~60%。男性、吸烟者(尤其男性)以及甲状腺肿大≥80g的患者的缓解率较低。TRAb的持续高水平以及多普勒彩超显示甲状腺血流增加与复发率高相关。131I和手术治疗的缓解率高于ATD,复发率低于ATD,但是,永久性甲减的发生率高于ATD。偶见没有及时发现和积极治疗的甲亢导致机体多系统受损,特别是心脏受累,或由于某些诱因导致甲状腺危象发生,可以导致死亡。

GD 的自然病程。GD甲亢的自然病程多样,难以预测。Wiersinga认为GD自然病程有如下特点:不缓解的病例占<10%;反复发作的病例占60%~70%;仅有一次发作的病例占30%~40%。对Graves病20年的随访研究发现,27%的患者治愈;62%的患者反复发作;8%患者亚临床甲亢;3%患者发生甲减[85]。有些患者甲亢,在几年之内变为甲减,然后又转变为甲亢。
