OSAHS患者经个性化口腔矫治器治疗前后心理状况改变的临床研究
【摘要】
目的：采用症状自评量表（SCL-90）评价阻塞性睡眠呼吸暂停低通气综合征（OSAHS）患者戴用个性化口腔矫治器前后心理状况的变化，并研究患者治疗前SCL-90各部分得分与多导睡眠监测（PSG）结果的相关性。
方法：经PSG监测及纳入、排除标准筛选出的44例轻、中度OSAHS患者，行个性化口腔矫治器治疗，并采用SCL-90问卷对患者戴用口腔矫治器前1周及戴用后3个月心理状况进行调查，对所获资料进行均数比较、配对t检验、两独立样本t检验及Pearson相关性分析。
结果：轻、中度OSAHS患者AHI、LSaO2、SCL-90问卷得分在治疗前1周及治疗后3个月的差异有统计学意义（P<0.01）；个性化口腔矫治器治疗的客观有效率为84.09%，OSAHS患者治疗前SCL-90躯体化、强迫症状、人际关系敏感、抑郁、焦虑、恐怖、偏执、精神症状差异具有显著性（P<0.01）。戴用口腔矫治器3个月后，OSAHS患者心理状况得到了有效改善，其中：躯体化、强迫症状、人际关系敏感、抑郁、焦虑、恐怖、精神症状改善差异具有显著性（P<0.01）。
结论：个性化口腔矫治器治疗OSAHS具有良好的客观疗效，患者的心理状况较正常人差，经个性化口腔矫治器治疗后心理状况得到明显改善。

Abstract
Objective：To study the Psychological state of obstructive sleep apnea hypopnea syndrome patients and the Effect of Self-adjustable oral appliance treatment，and to study the correlation between self-rating scale（SCL-90）and the result of the polysomnography（PSG）examination.
Methods：44 patients were diagnosed mild，moderate OSAHS monitored by PSG，use SCL-90 to analyze and contrast psychological state before and after treated with an Self-adjustable oral appliance，and conducting mean comparison，paired t-test，two-sample t-test and Pearson correlation analysis on the information obtained.
Results：The changes of mild and moderate patients’apnea hyponea index（AHI），lowest blood oxygen saturation（LSaO2），and SCL-90questionnaire scores between a week before and the three months following wearing Self-adjustable oral appliances are of statistical significance（P<0.01）；the objective response rate of the treatment with Self-adjustable oral appliances is 84.09%，The scores of somatization，enforcement，interpersonal sensitivity，depression，anxiety，Terror，paranoia，psychoticism of SCL-90 in OSAHS were higher than average scores in China（P<0.01）.The somatization，enforcement，interpersonal sensitivity，depression，anxiety，Terror，psychoticism scores obviously decreased after treatments druing theree month（P<0.01）.
Conclusion：Wearing Self-adjustable oral appliances has positive objective curative effects on OSAHS.OSAHS patients are more anxious and depressive than normal.After treatment with Self-adjustable oral appliance is psychological state are obviously improved.
