第十一章 同 情 心

【点评】此话看从哪个角度说,面对痛苦与生死,如果忘掉自己医生的角色与普通人和家属一样同悲切,一方面医生身心不堪重负,更重要的是不利于医生保持应有的理智与清醒履行救死扶伤的天职。所以英国著名心脏外科医生斯蒂芬·韦斯塔比(Stephen Westaby)强调:“如果有患者死亡,我们只能接受,不能被影响”。面对苦难,一名称职的医生如何做到既不冷若冰霜,又不能乱了阵脚,这个度的把握需要修炼。

8.No physician, sleepless and worried about a patient, can return to the hospital in the midnight hours without feeling the importance of his faith.The dim corridor is silent, the doors are closed.At the end of the corridor in the glow of the desk lamp, the nurse watches over those who sleep or lie lonely and wait behind closed doors.No physician entering the hospital in these quiet hours can help feeling that the medical institution of which he is part is in essence religious, that it is built on trust, no physician can fail to be proud that he is part of his patient's faith.

10.Physicians are the natural attorneys of the poor and social problems should largely be solved by them.

【译文】医生是穷人的自然辩护人,社会问题在很大程度上应该由他们来解决。

【考注】让我意想不到的是这句话竟然出自德国威名赫赫的病理学家Rudolph Virchow,更让人震惊的是,1848年6月,Virchow曾创办过一份名为《医改(Medical Reform)》的周报,旨在推广社会医学,其第一个口号就是“Physicians are the natural attorneys of the poor”。不幸的是,一年后,迫于政治压力关闭了这个刊物,Virchow被开除公职,流放到政治腹地。

尽管现实中医生不可能是普救众生的施主,但社会和患者总是有意无意之间会对医生有这种天使般的苛求。170年后的今天,美国加州大学尔湾(Irvine)分校的医学院副院长,医学、生物化学、公共卫生和流行病学教授Frank L.Meyskens博士在他的博文“The Physician Is the Natural Attorney of the Poor”: White Coat Conversations.(Frank L.Meyskens, MD@FrankLMeyskens Jan 28, 2015)中写道:如今这一祸患再次周期性地全面复燃,所有社会公平的重要指标都存在严重的不平等和偏见:健康、财富、司法和正义推动了这场祸患的复燃。并进一步呼唤身处这种环境,作为医生这个职业和医学本身应该参与进来为穷人代言,因为它正处于风险之中,治疗和同理心正在成为一种商品。

身处太平洋彼岸的我们正在经历一场新的、更深刻的医疗改革,恐怕我们也不能作壁上观。
