结肠癌性梗阻的Ⅰ期手术治疗

摘 要
目的：探讨结肠癌致结肠梗阻的Ⅰ期手术处理措施。
方法：对1997～2002年间46例结肠癌性肠梗阻的外科治疗进行分析。
结果：Ⅰ期手术35例，其中右半结肠切除9例，横结肠癌切除6例，左半结肠切除20例；Ⅰ期行左半结肠或乙状结肠直肠交界处切除、近端结肠造瘘，Ⅱ期闭瘘9例；Ⅰ期造瘘或回结肠吻合捷径手术2例。术后并发症发生率6.5%，围手术期病死率4.3%。
结论：结肠癌性梗阻 Ⅰ期肠切除吻合手术，应严格掌握适应证，并加强围手术期处理，才能提高疗效，减少并发症。

Primary surgical treatment of intestine obstruction caused by colon carcinoma.

ABSTRACT
Objective:To study the primary surgical treament of colon carcinoma complicated colon obstruction.
Methods:Data of 46 cases in our hospital from 1997 to 2002 was analyzed.We evaluated the methods and the effect of the primary surgical treatment.
Results:There were thirty-five cases of primary surgical treatment，The operative complication rate was 6.5%o.The mortality was 4.3%.
Conclusion:Complected evaluate the patients of colon carcinoma and surgical adaptation，the primary colectomy and anastomosis may be promote effect and reduce operative complications

结肠癌出现典型肠梗阻时多为晚期，临床处理颇为棘手。本文报告我院1998～2002年间收治46例结肠癌并肠梗阻病人的临床资料，对Ⅰ期手术治疗进行重点讨论。
