经胸乳入路腔镜甲状腺切除术影响美容效果的因素分析

摘 要
目的：探讨影响腔镜甲状腺切除术美容效果的因素。
方法：为43例患者采用经胸部乳晕入路行腔镜甲状腺肿瘤切除术，其中甲状腺腺瘤26例，结节性甲状腺肿16例，甲状腺癌1例。
结果：41例手术获得成功，26例行单侧甲状腺包块切除术，15例行甲状腺腺叶大部切除术。2例中转开放手术。手术时间80～140 min，平均100.5 min。1例发生皮肤瘀斑，1例皮下积液。术后住院5～7d，平均6.3 d，患者对美容效果满意。
结论：内镜甲状腺手术的美容效果与术前评估、手术操作方法及术后处理有关。

The factors impacting on the cosmetic effect of endoscopic thyroidectomy via breast approach

ABSTRACT
Objective:To explore the factors impacting on cosmetic effect in laparoscopic thyroidectomy via breast approach.
Methods:Laparoscopic thyroidectomy via areola of breasts was performed in 43 patients，including 26 cases of thyroid adenoma，16 cases of nodular goiter and 1 case of thyroid cancer.
Results:The laparoscopic thyroidectomy was successfully carried out in 41 cases，ineluding dissection of unilateral lobe tumor in 26 cases，subtotal resection of thyroid lobe in 15 cases，2 cases were converted to open surgery.The operative time ranged from 80 to 140 min（mean 100.5 min）.Skin ecchymosis was found in one case，and subcutaneous hydrops occurred in 1 case.Postoperative hospital stay ranged from 5 to 7 d（mean 6.3 d）.All the patients were satisfied with the cosmetic results.
Conclusion:The cosmetic effect of endoscopic thyroidectomy associates with preoperative assessment，operation method and postoperative management.
